What is a Patient Centered Medical Home (PCMH)?

A patient centered "medical home" is an approach to providing primary care to people of all ages.  It is not a building or place, but a team of health professionals who work together to provide a central point for coordinating care to help people become as healthy as possible.  For more information and to download the PCMH brochure, click HERE.

Why a Patient Centered Medical Home (PCMH) Model?

There has never been a better time to introduce a patient centered medical home (PCMH) model in Maine.  Good primary care can improve health outcomes and lower health care costs by increasing the likelihood that patients will get standard preventive care and manage their chronic diseases. We desperately need a new model from primary care. PCMH offers an exciting option because it changes how we deliver and pay for care and how patients experience the care they receive.

The Maine Patient Centered Medical Home (PCMH) Pilot

Recognizing the essential role of primary care in our health care system, the Dirigo Health Agency’s Maine Quality Forum (MQF), Maine Quality Counts, and the Maine Health Management Coalition work together to lead the Maine Patient Centered Medical Home (PCMH) Pilot. The participating pilot practices include a diverse mix of adult and pediatric practices from around the state that were selected for their demonstrated leadership for and commitment to the principles of the PCMH model.

As part of their participation in the pilot, practices are expected to implement a set of ten “Core Expectations” addressing key practice changes, and will be supported in their continued efforts to transform to a more patient centered model of care through participation in a PCMH Learning Collaborative. The ultimate goal of this effort is to sustain and revitalize primary care both to improve health outcomes for all Maine people and to reduce overall health care costs.

What is a Health Home (HH)?

The MaineCare Health Homes (HH) initiative began as an effort to bring the patient centered medical home model to primary care practices in Maine who participate with MaineCare (Medicaid). In partnership with MaineCare and the Department of Health and Human Services (DHHS), Quality Counts supports participating practices in the Health Home Initiative by providing them with education, tools, and informative resources to help practices achieve and maintain a truly patient-centered, high value delivery of care.  To read more about the MaineCare Health Homes initiative, click here
