QC Health Improvement Partnership Leadership Group
The mission of QC Health Improvement Partnership Leadership Group (QCHIP) is to provide operational guidance, advice, and direction for Maine Quality Counts adult health improvement collaboratives and initiatives. This leadership group will ensure that the initiatives are successfully implemented to support the goals for efforts to improve care and population health outcomes for patients and assist in building a culture of health throughout Maine.
Specific issues and tasks to be addressed by the Leadership Group include, but are not necessarily limited to the following:
- Assist in building a culture of health throughout Maine through networks of interested individuals and organizations to share experiences and to learn from one another, encouraging the growth of linkages between clinical settings and optimizing community resources to support patients.
- Provide input on strategies to promote community linkages between the initiatives and the opportunity to optimize community resources to engage patients.
- Project Evaluation – Evaluation of these initiatives will include evaluation of changes made by participating practices, and the impact of those changes on care processes and outcomes. The QC Health Improvement Partnership Leadership Group will be asked to advise on the structure and process the project evaluation, in conjunction with the project evaluation team.
Members are comprised of a multi-stakeholder group representing health care provider associations, consumer organizations, employers, payers, and other key stakeholder groups with an interest in improving health care and population health outcomes for patients.
QC adult health improvement collaboratives and initiatives:
