PCMH Learning Collaborative & Learning Sessions

 

The Maine PCMH Pilot is offering support to practices in the Maine PCMH Pilot through a Learning Collaborative modeled after the successful “Breakthrough Series Collaborative” (BTS) model developed by the Institute for Healthcare Improvement (1, 2).  As part of the application process, Pilot sites agreed to participate in the Collaborative with a “leadership team” made up at a minimum of a physician leader, clinical support, and administrative leader. The Collaborative will be led by faculty and staff experienced in practice change and will include a series of three “learning sessions” each year that bring teams together with faculty and content experts to promote collaborative learning and rapid cycle improvement methods.

 

There have been eight Learning Sessions to date.

Learning Session 2
- Feb 12, 2010 (Maple Hill Farm, Hallowell) - Over 130 participants from the twenty six Pilot teams met on February 12th to revisit issues of leadership, teamwork, and change management, while additionally focusing on enhanced access and practice-integrated care management.

Learning Session 4 -  October 22, 2010 (Governor Hill Mansion, Augusta, ME)-Twenty six primary care practices, their patient & family advisors, and several community partners attended the fourth Learning Session of the Maine PCMH Pilot on October 22, 2010.  The session had the best attendance yet with over 160 people participating in the event. The focus of the session was on engaging patients and family members in practice and on connecting with community partners on behalf of patients.

On Sunday, October 24, the Kennebec Journal published an article about the Maine Patient Centered Medical Home (PCMH) Pilot. Reporter Mechele Cooper interviewed providers and staff members to help readers learn more about this exciting approach to care. Learning Session 6  - June 3, 2012 (Hilton Garden Inn, Auburn) This learning session focused on reducing avoidable health care costs. Learning Session 7- October 21, 2011(Hilton Garden Inn, Bangor: This session focused on the aspects of Leadership and Teamwork required to accomplish and sustain transformation. The day-long Session provided opportunities for the Pilot practice teams to learn more about steps they can take, in collaboration with their “medical neighbors”, to address the important task of working together to accomplish and sustain transformation.  One of the speakers, Jeffrey Brenner, MD, shared his experiences with addressing the needs of high needs populations - specifically, the attributes needed by every level of leadership to accomplish the work. This session was titled, 'Harvesting the Power of the Team'.

Learning Session 8 - February 10, 2012 (Augusta Civic Center):  This session was held in collaboration with the Community Care Teams and First STEPS programs.  There were 300 attendees at the combined event which featured several national speakers and focused on Best Practices for Improving Access & Coordination of Care.

Practice QI Coaching

PCMH Pilot practices also recieve one-on-one quality improvement (QI) coaching provided by external, experienced QI staff from PHOs and provider organizations around the state. Several PHOs have lent their support to this effort, and are providing experienced QI coaching to support many of the Pilot practices; additional funds have been secured from Harvard Pilgrim Healh Care to support QI coaching for non-PHO supported practices.  See the list of specific QI coaches working with each Pilot practice:  PCMH Coach list 2012
