Improving Ambulatory Care

This brochure gives a brief description of the online program, "Better Choices, Better Health", which helps people manage chronic conditions.  This program is based on the program developed by Kate Lorig at Stanford University sometimes referred to as the "Chronic Disease Self-Management Program".

If you care for children with asthma, make sure that you order the latest asthma action plan which was developed in collaboration with the Maine Asthma Council for use with school age children.  The new plan is designed for easy use at the practice, home and school.  Packs of 25 plans can be ordered at www.jsmccarthy.com/mainehealth if you are registered with J.S.
