Improving Chronic Illness Care

Helping improve the quality of care for people with chronic illness is an important focus of Quality Counts.  At this time, we have developed tools to help people with diabetes better understand their condition and know what questions to ask their doctor.

Maine Quality Counts is working to identify tools for other chronic diseases, such as cardiovascular disease, asthma,  and depression.  Please check back soon for more information and tools to help patients with these conditions.
