Application Process Re-Opened for Maine PCMH Pilot Expansion and MaineCare Health Homes Initiative

Conveners of the multi-payer Maine Patient Centered Medical Home (PCMH), the Dirigo Health Agency’s Maine Quality Forum, Maine Quality Counts, and the Maine Health Management Coalition, have announced plans to expand the Pilot to include up to an additional 50 adult practices in January 2013.  Pilot conveners have worked with MaineCare to develop a single online application for participation in the Phase 2 expansion of the multi-payer Pilot as well as the MaineCare Health Homes Initiative.  Deadline for completed practice applications has been extended until Friday, May 4th.
