Patient Centered Medical Home

Recognizing the essential role of primary care in our healthcare system, the Maine Quality Forum (MQF),Quality Counts, and the Maine Health Management Coalition are working together to lead the Maine Patient Centered Medical Home (PCMH) Pilot. Following an initial planning period, the group selected a group of 26 primary care practices in May 2009 to implement the PCMH model as a first step in ultimately achieving the goal of statewide implementation of a patient centered medical home model.

Assessing Practice Readiness

There are a number of tools available to help practices measure their progress implementing the PCMH model and to document the structures and processes that are in place.

Medical Home IQ - MHIQ

The MHIQ is a free, web-based self-assessment tool developed by the AAFP’s TransforMED initiative designed to let practices assess the degree to which they have developed the structures and processes that support the medical home model. The MHIQ is available at TransforMED MHIQ (click on the Medical Home IQ link to start the assessment). The MHIQ provides a cross-walk to the longer (fee-based) NCQA PPC-PCMH tool.

NPR Series Focuses on Primary Care and the Medical Home

Last month, NPR health policy correspondent Julie Rovner visited Maine and interviewed several primary care providers about the pressures of primary care and potential solutions, and has featured the story in a three-part series on All Things Considered titled "Primary Care under Pressure". NPR staff interviewed providers in three different primary care settings, and included discussion with Dr. Lisa Letourneau of Quality Counts highlighting the Maine Patient Centered Medical Home as one important approach to addressing the crisis in primary care. The series is being broadcast on NPR radio stations on Aug. 25, 26, and 27. You can view transcripts and listen to broadcasts from the series on the NPR website.

Maine PCMH Pilot to Add 20 New Practices!

Maine PCMH Pilot Announces Plan for Phase 2 Expansion

Conveners of the multi-payer Maine Patient Centered Medical Home (PCMH), the Dirigo Health Agency’s Maine Quality Forum, Maine Quality Counts, and the Maine Health Management Coalition, announce plans to expand the Pilot to include an additional 20 adult practices in January 2013.  An online application for participation in this Phase 2 expansion of the multi-payer Pilot is now available.

 

MaineCare, Maine’s Medicaid program, has been an active supporter of the Maine PCMH Pilot from the outset and currently participates as a payer in the Pilot. MaineCare is now broadening its support for practices that agree to meet medical home criteria and commit to the PCMH Pilot Core Expectations through its new “Health Homes” initiative, an effort authorized by Section 2703 of the Affordable Care Act and a key component of the Maine Department of Health & Human Services’ “Value Based-Purchasing Strategy”.

 

More information on the Maine PCMH Pilot Phase 2 Expansion and plans for the MaineCare Health Homes initiative is available in the ME PCMH Pilot Expansion & MaineCare HH_Summary_02-12 and in a slide set, ME PCMH Pilot Expansion & MaineCare HH_Slides_ppt_02-12.  Interested practice sites are also advised to read the full ME PCMH Pilot Phase 2 Expansion Notice_01-11-12 and to review the Eligibility Criteria - PCMH Pilot Expansion & Health Homes prior to proceeding with an application.

 

Eligible practices should complete an online application by March 31, 2012.  Practices are strongly encouraged to review a PDF version of the Practice Application - Maine PCMH Pilot Expansion & Health Homes prior to completing the application online. As part of the application, practices must also complete and submit a signed Memorandum of Agreement (MOA - see Maine PCMH Pilot Practice MOA_Practice Expansion)

Timeline for Pilot Practices

•    January 12, 2012 – Pilot Conveners communicate plans for Phase 2 expansion broadly within state
•    January 2012 – Interested practices are asked to submit online Intent to Apply.
•    February 1, 2012 –Maine PCMH Pilot Phase 2 practice application posted online
•    March 31, 2012 - Deadline for practices to submit online application
•    April – May, 2012 – Review of applications by Pilot staff, PCMH Pilot Selection Committee
•    May 1, 2012 – Phase 2 Community Care Team (CCT) application posted online
•    May 31, 2012 -  Phase 2 practices selected
•    July 15, 2012 – Deadline for CCTs to submit online application
•    August 1, 2012 – Phase 2 CCTs selected
•    January 1, 2013 – Phase 2 practices and CCTs begin participation in Maine PCMH Pilot

Friday, February 10, 2012 marks the 8th Learning Session for the Maine Patient Centered Medical Home Pilot Practices at the Augusta Civic Center. This Learning Session is an important opportunity for teams both to refocus on some of the basic elements of the PCMH Pilot - i.e. improving access and improving coordination of care across settings - and benefit from new opportunities for collaborative learning.

Support for Practice Transformation

The Maine PCMH Pilot offers support to practices participating in the Pilot for their continuing efforts to transform to a more patient-centered model of care.  We recognize that many primary care practices in the state have already demonstrated signficant leadership for change, and have made considerable committments to improving their office systems to deliver higher quality and value.  Through the PCMH Pilot, we hope both to provide direct support to practices participating in the Pilot, and to share "lessons learned" with other interested practices around the state.  Direct support for participating practices is offered through both a "PCMH Learning Collaborative", and a network of "Quality Improvement Coaches" - individuals with quality improvement (QI) experience working directly with participating practices to help them assess their current capacity and make ongoing improvements in care.
