The Maine Patient Centered Medical Home (PCMH) Pilot is a multi-stakeholder effort convened by the Dirigo Health Agency’s Maine Quality Forum, Quality Counts, and the Maine Health Management Coalition to implement and evaluate the PCMH model as the first step in achieving its statewide implementation. Practices participating in the 3-year Pilot confirmed their commitment through a written “Memorandum of Agreement”, which outlines the anticipated benefits and expectations for primary care practice sites participating in the Pilot, and includes a set of “Core Expectations”, listed in the attached document.
