Living with Chronic Conditions
Managing any chronic health condition is challenging. But spending a few minutes every day doing the right things can make a lifelong difference in your health. TIP: Start with small steps, and as these become easier and part of a daily routine, add another one. You don't have to change everything at once. You can do it!
On the left side of this page, you can find tools and resources to help for some of the most chronic conditions, including diabetes, cardiovascular disease, asthma & COPD, high blood pressure, and managing your weight.
There are also programs to help people make the types of changes needed to better manage many chronic conditions, including becoming more active, eating healthy, managing stress, and taking medicines regularly. One of these programs, called "Living Well" is offered both through in-person, group education sessions held in many communities around the state, and through a web-based, on-line program that you can do from your own home.

For more information about the online program, see the Better Choices, Better Health Program Brochure. Better Choices, Better Health is a free, six-week, interactive workshop for people with chronic conditions. It can be completed from any computer with an Internet connection, including dial-up. Interested individuals can register at SelfManage.org. Spaces are limited, however, and availability is on a first-come, first-served basis.
